Vision Care Program

  1. Vision Care Reimbursement Forms can be obtained in the Human Resources Office or on the college’s website.
  2. Vision Care Reimbursement Form is completed by the employee. The completed form along with all receipts are submitted to the Benefits Administrator.
  3. Documents are reviewed for eligibility-based program criteria.
  4. Employees can expect reimbursement within 30 days from submission of claim form.
  5. Claims not eligible for reimbursement will be returned to the employee with an explanation for the denial of the claim.
  6. Inquiries should be directed to the college’s Benefits Administrator.
